Going Home

Checking Out and Discharge
To prepare for your discharge, the staff will work with you to help you get ready for your return home. You will receive instructions about follow-up care, such as diet, medications, level of activity and follow-up appointments with your physician. A nurse and physician will review these instructions with you. Please be sure to ask them about any concerns you may have. If you have any special needs, such as home health care or home medical equipment, the Case Management department will assist you with these needs during the discharge process. Please call them at (253) 804-2835, if you have any questions.

Because the hospital admits and discharges many patients every day, your room will be assigned to another patient shortly after you leave.
